We have been updating our forms!

The Annual Membership Form and the accompaning Assumption of Risk Form are now available in a single, double sided pdf document. We have also made it so that spouses/significan others can sign on the same form as the member.

The GUEST Assumption of Risk Form now includes a place to identify the Member.

Annual Membership Form AND Risk/Indemnity Form (2-page document) - Note: If you don't plan on coming into the Office to retrieve your 'new' member card and will be mailing with a check, include a stamped, self-addressed envelope. The card will then be promptly mailed to you!	Download
